The Pathways to SmartCare team will analyze your specific population and tailor a comprehensive wellness program that best suits the culture of your business environment. Program communication promotes the attendance of chosen events and programs. Health Risk Assessments (HRA) and feedback reports monitor the effectiveness of the program and maximizes your Return on Investment. The first step in assimilating a wellness program into your workplace includes using a Health Risk Assessment (HRA). An HRA is an assessment tool used to measure the health of a particular population. The data then generates Personal Health Guide and a Corporate Aggregate Report for group analysis. Personal reports are sent directly to the employee’s home, and at the request of the employee, to their personal physician. Our process ensures 100% confidentiality. 2. Health Screenings are provided on-site through Pathways to SmartCare in conjunction with the HRA. Blood Glucose, Cholesterol, Bone Density and Blood Pressure are a few of the screenings available. These employee specific measurements are then used within the HRA to provide a more accurate benchmark for individual employees. Early detection is crucial to the ability to treat an illness! 3. Health and Wellness Events are key when beginning a wellness program with Pathways to SmartCare. An annual kick-off health event provides opportunities to personally meet with health and wellness providers who display information about their products and services. Combining health screenings and the HRA at an event is a great way to communicate the value of the Pathways to SmartCare wellness program to employees. Pathways to SmartCare will research specific needs of the population to develop an appropriate mix of topics and exhibitors to include in the event. The project team will also organize the logistics of the event, including layout, access to electricity, tables and chairs, and a representative will attend the event to ensure all aspects of the event are managed. 4. The next step of the wellness plan is continuing the learning and growing process with your employees. There are several ways to reach the employees and continue the mission of the wellness program. Some employees respond quickly to group activities, while others prosper individually. The following techniques drive the success behind Pathways to SmartCare. Wellness Focus Groups Employees attend formal gatherings led by health care providers. The group meets for 6 to 8 weeks to receive information and take part in discussions on specific disease states. Lunch and Learn Health care professionals guide employees through lunch time lectures designed to educate employees to take control of their health and wellness at work and home. One-on-One Counseling Personal counselors are available via phone, email or in person for those employees requiring one-on-one assistance in reaching their wellness goals. Guided Self Help Personal health care, weight management, stress release and other health care materials help employees make more confident health decisions. |
